Both are worth ruling out. Thyroid problems, certain medications, caffeine, and sleep disorders can all produce symptoms that look like anxiety, which is why the evaluation covers medical history rather than symptoms alone.

Often it helps. Medication and therapy work on different parts of the problem, and for many people the combination outperforms either alone. If you already have a therapist, care can be coordinated with them.

Long-standing anxiety is one of the more treatable things in psychiatry. A long history is not evidence that treatment will not work — often it is just evidence that it was never properly tried.

That is still worth an appointment. Part of the evaluation is working out how much is situational, how much is a treatable condition, and what actually helps in each case.
